A national response to paedophilia and child murder(d20)
In August 1996, Belgium was deeply shocked to learn of the rape and
murder of several young girls by a group of paedophiles. In the wake
of these events, the Belgian population displayed symptoms of
collective emotional shock and bereavement. We endeavoured to
come to terms with these feelings as a community. We immediately
sent an open letter to all children, which was published in the
country's main newspapers. We then organized a group debriefing on
national television on a very popular children's programme. This
article deals mainly with the processes involved in this debriefing
session. Further complications later developed involving, among
others, abused children and their families. In order to help them,
in addition to individual interventions, we again used the press
and radio and TV programmes. When the population eventually began
to emerge from the shock and bereavement, we acted to prevent
excesses by, among other things, writing an ' open letter to child
abusers '.

About joint custody
The joint custody may be a positive application of shared parental
status, but it is not an obligation.
This paper describes the desirable conditions to the setting up of
a joint custody ; they concern as well, the child as the parents’
motivations and the state of mind that will exist once the project
set up ; they also concern the practical parameters ( nearness of
official domiciles ). The article ends up with a brief evaluation
of the results.

A case of solid food refusal by a thirty months old
boy
A case of solid food refusal by a thirty months old
boy was successfully treated in five sessions.
To ensure continuity of child and parents therapy
sessions, they were asked to pursue at home the
techniques elaborated with the consultants. Parents
were able to cooperate with efficiency and helped the
child differentiate between fantasy and reality.

About a severe and collective post-traumatic stress syndrome
in Belgium.
- Following the events of August 1996 ( rape and
murder ofchildren ), the Belgians have brutally experienced a
syndrome of severe post-traumatic stress and acute grief,
provoked by the loss of young children identified as loved
ones, by the diffuse anxiety of new aggressions, as well as
by the repeated evidence of the negligence and rivalries of
supposedly protecting officiai institutions. The Mental
Health officials present in Belgium at this time have
suftered as everybody else and have set up manifold forceful
measures aiming at treating the others'traumatic neurosis
as well as their own. The originality of part of these
measures is that they had a collective purpose so as to
produce a powerful multiplier effect. Affenvards, the Mental
Health authorities have also had to face other foreseeable
consequences of this community stress as, for example, an
upsurge of the suffering in parts of a population already
fragilized by sexual abuse, or the appearing of contagion
phenomena in lovesick teenagers or else, paradoxically, the
designation of scapegoats among the children and adolescents
themselves. Later on, the Belgian population has stood up
again and informal movements of citizenship have arisen
around the desire to fight directly or indirectly against
sexual abuse and there again the role of the Mental Health
institutions has consisted in backing up these movements with
solidarity and consideration.

Perverse sexual activities stricto sensu exist
among childrn, although this is the least
frequent category ( far behind normal-
developmental sexual activities, sexuality
generated by fear, etc. ).
There are three mechanisms that account for te
appearance of these activities : the fact that
the child learns how to seek intense pleasure,
which it found first by chance ( in the broad
sence of the term ) compensation-denial when
dealing with sexual encounters experienced as
frightening and of which the idea is repressed
( an explanation put forward by conventional
psychoanalysis ); repudiation of sociocultural
norms and/or natural laws ( sometimes, in a
diffuse manner, with the complicity of a parent
) ( an explanation put forward by Lacanian
psychoanalysis).
The main clinical indicators are : the bizarre
nature of the sexual act; the intensity of
pleasure felt without fear or guilt; the state
of " withdrawal " pending the perverse act; the
subtle cooperation of intelligence; and if
another person is involved, failure to take
into account his or her human dignity ( to the
point, occasionnally, of perpetrating violence
against him or her ).
In order to be able to help and treat such a
child in this phase of its sexuality, we must
be able : to observe it with vigilance and
discretion; to challenge our notions as adults;
to dialogue with the child on what it has done
and the results of its action; to forbid any
recurrence; to arrange for greater presence and
appeal in its life.

The article describes why some children
fiercely refuse to have any contact with one of their
parents during a long period of time, after their
separation. It also depicts serious phenomena that are
sometimes bound to this refusal ( child abduction,
allegation of child abuse, etc.).
Then, we evoke the risk of reducing these complex
phenomena to the only expression of " parental alienation
syndrome ", as the Northern-American psychiatrist R.
Gardner and his school have promoted it.
Finally, the article explains in details ways of behaving
when confronted with such refusal situations, which
often have important systemic implications. Each times
adapted to original situations, they most often include
psychological, social and judicial components and require a
lot of energy, coordination and persistence.

Every evening, she could not go to sleep unless she had settled

partially brought on by anxious and depressive ideas ans sustaines
through the regressive pleasures it procured her.
The therapist proposed treatment for Catherine on the one hand and
for her parents on the other. Without jolting them, he helped
reflect on the symptom's meaning, and on the pros aud cons of
keeping it. The symptom disappeared after a year's work.

Child Psychiatry Emergencies : Analysis and Specificities.
We have noticed that the topic of psychiatric
emergencies of the child hasn't
been considered much up to now. However
these emergencies exist. We have therefore
tempted to redefine this concept and have
analysed the usual reasons for which certain
child's psychiatric emergencies are " forgotten "
( this raises implicitly the reflection on
the actions to be led in order to rectify these
lacuna ).
Then we propose a table inspired by the
clinic. It allows the drawing of global significances,
structures and " etiopathogenic "
classes of psychiatric emergencies of the
child.
Finally, we try to determine globally the
specific aspects of an emergency pedopsychiatric
intervention for children according
to our ideas.

Parental Function for Hyperkinetic Children's' Management.
The author first describes the numerous possible etiologic factors,
implicated in child hyperkinetic behavior.
Then, he writes about the parents important function to take
charge of the problem. Their therapeutic contribution can be
decisive, but requires a lot of energy ; Thanks to a psychosocial
guidance, they can find new attitudes that reduce the child's
anxiety and negative self-esteem. They also can better educate
the expression of his pulsions and improve his psycho-motor
control. Topics such as family rules, negative and positive
sanctions, and indifference have to be worked out.
The child's teacher can be associated to this work, as well as the

Psychopedagogical guidance for parents of children with autism.
The authors investigate the place given to the parents in the
treatment plan for children presenting pervasive developmental
disorders, especially autistic troubles and propose the notion of
psychopedalogical guidance. This denomination includes an
introspective dimension in the work with parents, where listening
has a prominent place, as well as a reflection based on more
behavioral methods. The authors describe the contents of these
encounters along four guidelines : containment, psychic experience,
daily life and pedagogical attitudes, as well as the need to continue
interacting with the child as a " subject ". They detail the four
aspects of their work and illustrate them through the use of clinical
examples.

The article first defines the sexual abuse on
minors and discusses the limits of the
definition. After that, it describes the
psychological functioning of the protagonists
of the abuse, the indicators of this one, and

pornographical material on the web. Most
children escape almost uninjured from
vizualisation of pornoqraphy. However some are

The life experience involving the siblings of mentally disabled
children : from adaptation to mental disturbance
Abstract :
The Authors underline the recent interest for the psychic life
experience of siblings of children with mental disabilities,
in particular for autistic disorders. They describe the sibling's
function in the subject's psychic development. They analyse in the
domain of mental disabilities in general the different factors
influencing sibling's life: composition of the brotherhood,
type of disabilities and parental psychic life experience.
They underline certain characteristics more specific of autistic
disorders. Based on their clinical practice and on scientific
studies, they try to describe the different sibling's reactions.
They define two main solutions: resiliency and mental health
troubles. Between these solutions they describe a wide range
of more or less successfull stressing adaptations. They conclude
by the importance of considering the presence of brothers and
sisters in all kind of therapeutic work of child with mental
disabilities or autistic disorders.

psychotherapeutic work with young teenagers and
their families. The age category which is examined is
that of early adolescence, a moment in life of
profound disorganisation of identity. After describing
the importance of flexibility in the organisation of the
proposed psychotherapy and the materials used,
listening and its focus is described :
patience, empathy, selective interventions of
approval, encouragement or disapproval, warnings,
etc. The therapist's concern gains from addressing
equally the adolescent and his family, without
becoming the arbiter of their relational difficulties.
The author then reviews interventions
aimed at " rassurance ", which

his internal debates and gropings are

Children’s Complicated and Pathological Mourning
In this article, the authors investigate children’s
pathological and complicated bereavement.
They first define a few concepts and
talk about mourning, not only when associated
to death, but also in every situation of
separation perceived as an irreversible loss.
They describe briefly phenomenology and
etiopathogenesis. They approach somatic
mournings, and notions of repression, distress,
guilt, depression, shame and traumatism,
illustrated by clinical situations. They
mention protective and worsening factors.
They particularly underline the therapeutic
and educational support that can be given to
these children and their close relations.
In their conclusion, they emphasize the
role of family members and point out the
risk of excessive interference.

The long-term evolution of children and teenagers victims
of sexual abuse
Based on thirty-five years of clinical experience in the field
of the fight against sexual
abuse, the author offers his clinical observations on the
long-term becoming of the
victims. Factors of gravity or protection may be evoked, but
only by statistical
indications. Among other things, one should not underestimate
the problem of the secondary traumatization.
The author distinguishes five possible poles of becoming : the
healing ( probably
most frequent ) ; the maintenance of high post-traumatic
anguishes ; depression and
culpability ; anger and victimization and finally the
modification of the social-affective
project in the direction wanted by the author.

L'article propose également quelques
recommandations à propos de l'éducation des

et la capacité d'apprivoisement de chacun;
protection du territoire propre de l'enfant;
solidification de l'existence de la nouvelle
famille et pourtant, paradoxalement, respect de

The child psychiatrist facing children suffering from
disorders known as "functional" ... and their paediatricians.
The author takes stock of the possible collaboration between
child psychiatrists and paediatricians, mainly as regards
children labelled as functional or psychosomatic cases. He
distinguishes three main categories of situations, depending
on whether families and/or children are receptive, ambivalent
or hostile to the idea of admitting that psychosocial factors
might contribute to the onset of the children's somatic
dysfunction. Then, according to the cases, the interventions
implemented in liaison child psychiatry, take on particular
nuances, without ever leaving aside the importance of the
therapeutic alliance with the paediatrician. The case study of
an older child presenting abdominal pains and school
invalidation, illustrates the concrete aspects of the situation.

The children after the divorce.
The author, law expert, relates a few cases in
which the interests and the deep feelings of the
children were not taken into consideration when
their divorced parents dispute the care of these
children.
In proceeding from the children's pain, as it
appears in consultation in form of anguish and
guiltiness, the author proposes to the parents
concerned, other attitudes in behalf of morality
including the respect of the other and in behalf of
a psychological efficiency on long term.

Sexual abuses on minors.
The author gives a definition of sexual abuse on
minors, emphasizing it is more frequent inside
the family (incest) than outside. He describes the
counter-transference reactions this type of abuse
induces, especially in professional teams who
tend to put each other in a position of rivalry.
Next, he sketches the pathogeny of sexual abuse,
the clinical signs and the long-term effects.
The author deduces what should be the first
signals of sexual abuse and proposes a pattern of diagnosis.
At last he explains a model of management of
the crisis and the long-term close support of this
difficult situation.

First interviews. - After recalling some of the general options
which determine the framework of his work, the author
describes the successive stages of the first interviews. For each
stage, he refers to the interior attitudes which guide the therapist
as much as to the form of his interventions. often concretely
illustrated. The author reviews the following stages: « family
photographs », multifocal descriptions of transactions between
parents and children with their behavioural and affective
dimension ; empathic reformulations, i.e. re-expression by the
therapist of his main impressions, first in a punctual way, then in
a synthesis discussed with the patients, from which a working
plan is outlined for the future; the stage of options: option of
principle when the patients opt for the status quo or for a
change, and applied options when a choice is made among the
proposals of aid. The aim of this article is less to present an
original working method than to recall the importance of
respecting a rhythm in the work and the importance of a
coherence between the therapist's interventions and his implicit
experiences.

How to manage the confidences of the child, real or imaginary
as some by the one that receives them, in individual interview ?
How to choose between values some: the recognition of the right
for the intimacy; the educational duty, which often connotes
that the parents are informed; the duty of protection? The
article suggests a way which integrates the consideration of
these various parameters.

The article focuses on the maltreatment against children in
Belgium, and on the main social reactions to care it. Firstly, we
describe the evolution of the types of maltreatment revealed, up
to their current and omnipresent centration in the sexual field .
Then, we evoke the deep evolution of scientific knowledge;
practical methods of preventing and helping have changed a lot
too, among others in the direction of systemic conceptions and
interinstitutional cooperation. Finally, we make an
approximative evaluation of the results and mention some
important existing errors in the general social representations
and programmas; therefore, we conclude with some positive
recommandations.

The article defends the need to, structure the first steps to, follow
at the beginning of the intervention.
This major period should be managed by the establishment of models
based on indicators. These indicators must be analysed in order to,
find the best guidelines for the socio-therapeutic intervention.
The research was accomplished in Belgium under the Belgian legal
conditions. But it should be helpful and instructive for all
professionals regardless their country.

... There are lots of different models of family apart from the
traditional nuclear family; that is why we will name them with the
generic term restructured families. Do they satisfactorily
meet - to paraphrase Winnicot - the psychic needs of children that
live inside ?
It is obviously impossible to study every individual case and the
answer cannot be from the type yes or no : like each human
institution, these families bear specific failings and treasures,
some of them momentary, during a crisis or a fruitful moment, and
others more structural.
In the first of these two articles dedicated to this subject, we
will discuss the love offered in response to the child’s
expectations and then what happens to his self-confidence, which
is in process of formation.

First the author describes clinically the

In a second time, the article describes the
diary bringing-up, as well as the treatment of
child's anguish. The autor develops such themes
as : how to prevent the anguish's installation,
to avoid some jeopardised, to listen and
partage information, to encourage the child to
face what is afraid of, to simulate positive
imagination, to reinforce self-truth, to use
introspective psychotherapies or medication ...
